Fannie Bay Medical Centre is a modern and new mixed billing medical centre in Fannie Bay that offers patients high quality comprehensive health care. With male and female doctors and onsite pathology, the centre is open 6 days a week and has on site parking for patient convenience.

​Dr Mathushinee Mohanachandran

BMED/MD/ FRACGP

Dr. Mathushinee Mohanachandran is a General Practitioner with a passion for women’s health and mental health. Originally from Sydney, Dr. Mathushinee completed her medical degree at the University of New South Wales (UNSW) before gaining valuable clinical experience at both Westmead Hospital and the Children’s Hospital at Westmead.

She has since transitioned into general practice, where she combines her expertise in primary care with her dedication to supporting women’s well-being and addressing mental health challenges. With a compassionate, patient-centred approach, Dr. Mathushinee is committed to providing holistic care tailored to the individual needs of each patient.

Dr. Safi Ahmed
MBBS, FRACGP

Specializing in emergency care and mental health.

 

His training and career cover a diverse range of disciplines, encompassing whole family comprehensive care, including chronic disease management, especially Diabetes and its complications, Geriatric medicine, Renal medicine, and all other areas of General Practice.

Dr. Salah Khan
MBBS, FRACGP, DCH (Diploma in Child Health)

Dr. Salah Khan brings over 25 years of extensive experience in general practice, with a special interest in mental health. After completing his medical education at Karachi University’s Sindh Medical College in 1992,

 

Dr. Khan moved to Australia in 2008. Prior to his move, he also had a stint working in Montreal, Canada, where he gained significant
international experience.

Dr. Khan has a strong commitment to mental health. He is known for his compassionate approach and dedication to providing comprehensive care to his patients.

 

Starting July 2024, Dr. Khan will be joining the Fannie Bay Medical Centre, furthering his devotion to healthcare excellence in a new setting.

Appointments

For appointments, please call between 8.30am and 4.30pm weekdays, 10:00am to 1:00pm Sunday and 1:00pm to 4:00pm in some public holidays.
Business hours may change in the near future.

Appointment times are usually made at 15-minute intervals. Emergencies will always be given priority and our reception staff will attempt to contact you if there is any unforeseen delay, or the doctor has been called away.

CANCELLATIONS: Policy Update - Cancellation Fee

Our goal is to provide quality health care to all patients in a timely manner. No-shows, late arrivals, and cancellations inconvenience not only our providers, but other patients as well. Please be aware of our policy regarding missed appointments.
Our Policy:
Any cancellation or reschedule made less than 3 hours will result in a cancellation fee of $30.00.

If you are more than 15min late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply.

AFTER-HOURS

If you have an emergency and you need medical care you should always call 000, visit your nearest emergency department.

RECALLS AND REMINDERS

This practice has a system in place for recall and reminder e.g. results, pap smears and immunizations. If you don’t wish to be included, please advise your doctor at the time of your consultation. If participating, please ensure all contact details are up to date.

INTERPRETER SERVICES:
For patients who may require a translating service, please advise reception at the time of booking so a telephone interpreter can be arranged.

 

TELEHEALTH

Our practice provides telehealth for our existing patients only.
To be eligible for GP telehealth services patients must have had a face-to-face consultation with their GP or another GP in the same practice, in the 12 months before the telehealth service.
We do not accept new patients for telehealth consultations, including those seeking appointments related to COVID-19. This measure is to ensure that we can provide the highest standard of care and thorough assessment, which is best conducted in person for initial consultations.

PRACTICE FEE
Fannie Bay Medical Centre has changed to Mixed Billing Practice. Patients under the age of 16 and patients who hold a valid Pension Card, Concession Card, Healthcare Card, or DVA card will not incur any costs.
Our fees will vary depending on the time taken and the degree of complexity of each consultation. Private fees are required to be paid on the day of consultation.

An approximate $45.00 out of the pocket fee will incur for normal hours, and approximate $50.00 out of the pocket fee for after-hours.

Eftpos facilities are available for your convenience. We can assist you with your Medicare Rebate.

GP Management Plan, Health Assessments, and Vaccination Consults will be bulk billed.
We do not accept cash.

PRESCRIPTION
Our practice does not prescribe S8 medication to new patients. Scripts can be provided through telephone consult for existing patients only and will be given an option to use e-scripts, pick up or if it needs to be sent to the pharmacy.

 

REFERRALS​​​
To ensure that referrals are sent to the right practice, this practice has the following procedure:

  • Doctor gives referral to patient.

  • Patient brings the referral to reception.

  • Reception to fax the referral to the referred Doctor.

  • Referral is given to the patient to take to their appointment.

 

RESULTS

If your doctor has requested any tests such as X-Rays or Pathology, we strongly advise you to come back and discuss the results with your doctor. If your results are normal, you will not be contacted by us. If your doctor would like to discuss your results with you, we will generally ring you to make an appointment. Under no circumstances will reception staff report results to patients. Please be aware, test results are confidential and will only be given to you directly, unless you give permission for your doctor to communicate them to another person on your behalf.

TELEPHONE CALLS AND COMMUNICATION

Generally, your GP will be unable to speak with you while consulting with other patients. Your phone query will be handled by our receptionists who will pass the message onto the Doctor or Nurse on duty. We aim to return phone calls before the end of business the same day.

PATIENT HEALTH RECORDS

​A patient health record is a detailed, confidential document compiled by a health professional over a period on a person. It is stored securely in an electronic format. Its primary purpose is to:

  • identify a person accurately

  • record symptoms and signs

  • support diagnosis

  • Document management decisions.

 

Our receptionists will confirm on your arrival that we have the right person booked in. Staff will ask you to give your name, date of birth, and home address each time to ensure that we have the right patient.

Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and ensure this information is only available to authorize members of staff. Fannie Bay Medical Centre complies with the Privacy Act 1988 and the Australian Privacy Principles 2014 including the way we collect, store, use and disclose health information.

TRANSFER OF MEDICAL RECORDS

New patients can fill out a form to request medical records transfer. That form is faxed to the previous GP who will send the old medical records. We prefer to receive them in electronic format (XML, or HTML). Any records received as paper printouts will be scanned into patients’ records.

Existing patients' medical records can be provided, depending on how the medical record is. A fee of up to $30 in paper form or records can be stored in a password protected USB.

PATHOLOGY – Australian Clinical Labs

We have pathology on site for your convenience; most tests advised by GP’s are bulk billed. Please see the staff for any questions or assistance.

Your rights:

If you have a problem with any aspect of the service, you receive at our medical centre we would like to hear about it.

We take your concerns, feedback, suggestions and complaints seriously. Please speak to the Practice Manager or your doctor to discuss your concerns.

PATIENT’S RESPONSIBILITIES TO OUR PRACTICE​

Attending this practice involves a relationship with all staff. In order to maintain a healthy relationship both parties must respect each other and have a mutual trust.
Our practice tries very hard to provide our patients with a high quality of care and we aim to continually improve our systems and services to help us do it better to care for you. We will gladly listen to your suggestions and complaints and follow them up constructively, as this helps us improve. However, we will not tolerate the following behaviors from our patients:

  • violence or threatening behaviour towards any staff member

  • yelling at our staff

  • verbal abuse or aggressive behaviour to our staff on the telephone

  • verbal or physical abuse towards our staff

  • angry behaviour towards our staff

  • continue to not follow Dr’s treatment advice

  • continue to fail to pay your bills
